I, blanks* declare that I am 18 years of age or older, and consent to participate in the above activity. I declare that I can swim at least 50 meters in open water and can tread water in light clothing for a minimum of 3 minutes (otherwise a life jacket will be provided).

I do not suffer from any illnesses that render me unfit for this activity. I agree that if I suffer damage, death or personal injury as a result of participating in this Rowing/Outrigger Canoe/Dragon Boat activity; I shall not hold the Royal Hong Kong Yacht Club (RHKYC), the appointed officials, staff & employees liable for such damage, death or personal injury, and I make this agreement in full knowledge of the risks inherent in these activities.

I agree to abide by all rules and training for the correct handling and use of RHKYC equipment both on and off water, and agree to pay for any damages caused by my actions while using the equipment.

I understand that I grant to RHKYC, at no cost, the absolute right and permission in perpetuity to use my image and likeness in any media, including video footage, for the sole purposes of advertising, promoting, reporting and disseminating information regarding RHKYC activities.
